Services included in offer, specified by American Dental Association codes (D9230 for nitrous oxide inhalation, D0140 and either D0220 or D0270 for limited exam with a single digital x-ray, and D8090 for Invisalign treatment), are not guaranteed to be sufficient to provide definitive diagnosis and allow for the provision of treatment in all cases. New patient exam includes ADA codes D0150 and D0210. Dentist reserves the right to refer patients to a specialist or requesting additional diagnostic imaging or tests prior to finalizing treatment recommendation/s and subsequent rendering of treatment. Patient reserves the right to deny additional recommendations but accepts that doing so may result in inability of dentist to render treatment that meets the standard of care accepted by the profession and therefore may deny providing treatment if he/she feels in his/her professional opinion that it can not be done safely or to the minimum standard of care. Treatment is not guaranteed to be performed on the same day. Prophylaxis, commonly referred to in lay terms as "regular cleaning" can only be performed in the absence of periodontal disease. If you have insurance, we may elect to bill your insurance for a portion or all of the services. Financing subject to credit approval. Promotions can only be used one time each per patient.
